A meta-analysis of 23 studies (2,054 shoulders) confirms that ultrasound (US) is comparable to MRI in diagnostic performance, though results are highly operator-dependent.

  • Accuracy by location: Highest accuracy for biceps (0.93) and supraspinatus (0.83); lower for subscapularis (0.76).
  • Tear Severity: US is significantly more effective at identifying full-thickness tears (0.93) than partial-thickness tears (0.81).
  • Operator Impact: Radiologists showed markedly higher sensitivity for partial-thickness tears compared to surgeons (0.86 vs. 0.57).
  • US vs. MRI: Comparative data found no statistically significant difference in sensitivity or specificity between the two modalities for supraspinatus injuries.


The study demonstrate that ultrasound remains a highly effective, non-invasive alternative to MRI, provided it is performed by an experienced physician.
