A retrospective study of intradiscal PRP concentrations and their effects on discogenic pain outcomes.

A 2022 retrospective study by Lutz et al. found that intradiscal injections of higher-concentration (> 10x) platelet-rich plasma (PRP) significantly outperform lower-concentration (< 5x) preparations for chronic lumbar discogenic pain. Patients receiving the higher dose experienced a 3.4-point pain reduction and 46.4% functional improvement at 18 months, compared to only 1.7 points and 33.7% in the historical low-concentration cohort. This suggests that the total “dose” of delivered growth factors (such as TGF-β and PDGF) is a critical variable in regenerative spinal treatments. While the high-concentration group reported an 81% satisfaction rate, one reported case of spondylodiscitis emphasizes the continued need for strict aseptic technique during intradiscal biologic procedures.
